  • Patients who wish to participate will be issued a MyChart activation code during their appointment with their physician.   • For your security, your activation code expires after 2 weeks. If your activation code has expired or you did not receive one to activate your account, simply click the “Sign Up Online” button on the MyChart login page.   • Yes, you can. This is called proxy access. Pediatric patients: legal guardian/parent is required to turn in the proxy request to the childs physicians office; the office will review the proxy request and grant access. This allows a parent (or guardian) to log into their personal MyChart account, and then connect to information regarding their family member. Complete a Proxy Consent Form and return it to your physician’s office to request access to this convenient service. Adults can invite other adults to have proxy access to their MyChart account and can choose between read-only access or access to send messages and schedule appointments.
